Job Description:
A company is looking for a Member Health Assessor.
Key Responsibilities
Conduct health risk assessments and gather information from members
and stakeholders
Coordinate logistics to support care plan goals and assist with
health education
Maintain documentation and contribute to care plan development
based on assessment outcomes
Required Qualifications
High School Diploma or GED is required
Minimum of two years of experience working with at-risk populations
in community settings is preferred
Proficiency in Microsoft Office applications
Sensitivity and experience working within diverse cultures
Ability to work independently and in a team environment
